It is the referring clinician's responsibility to ensure that the application is completed as accurately and comprehensively as possible. This includes full copies of any supporting clinical research evidence. Incomplete forms providing insufficient information will be returned.

Commissioning Policies

This application is to be used for treatments which are not routinely commissioned and where there is evidence of exceptional clinical need. The individual funding request (IFR) panel cannot consider funding a person's treatment where there are others who would equally benefit.

An Individual Funding Request (IFR) is a request to fund treatments, drugs or devices under medical supervision for an individual (referred to as 'person' in this document) which is not routinely commissioned. Such a request must be made by a clinician or relevant professional.

NHS Kernow Clinical Commissioning Group (NHS Kernow) is committed to improving the health of people living in Cornwall and the Isles of Scilly, and to ensure that they receive the treatments they need at the right time, in the right place, to a high standard in order to give the best health outcome.

NHS Cornwall and Isles of Scilly Integrated Care Board must provide healthcare within the budget it has been given. That legal requirement means some hard choices have to be made.
